Job Description Job Description Position Summary: The Community
Health Outreach Advocate (CHOA) directly assists the Director of
Community Health and the Community Resource Center Manager in
advancing Community Care Plan’s mission to promote healthier
communities. Through outreach and engagement activities, this
position will assist community residents in linking to local
resources that address Social Determinants of Health (SDoH) and
improve access to healthcare. This position will engage with local
and faith-based organizations representing Community Care Plan and
through engagement with community members to improve their health
and well-being while creating brand awareness of CCP in the
community. Position duties will be primarily performed within the
assigned service area. Essential Duties and Responsibilities:
Assists community members in linking to local social service
agencies that address SDoH, such as housing, utilities, food, and
benefit programs. Accurately documents client interactions and
referrals in the electronic. Responsible for building and
maintaining positive working relationships with local social
service agencies on behalf of CCP. Assists community residents with
identifying and reducing barriers to accessing medical care.
Communicates with community members the importance of preventive
health care and managing chronic disease. Locates and engages CCP
enrollers who are not currently engaged in preventive or prenatal
care to identify and eliminate barriers, promoting linkage to care.
In communities where CCP has membership, the CHOA works with the
Concierge Care Coordination (C3), Customer Experience (CX) staff
and the CCP member to navigate plan benefits, provider links and
social services to reduce barriers and gaps in care. Models
commitment to continuous quality improvement and assists CCP in
projects and events as needed that address HEDIS care gaps.
Identifies and verbalizes to CCP the culture, norms, beliefs, and
preferences of their community to ensure CCP has a good
understanding of each community’s unique needs. Maintains active
communication with the Director of Community Health and the Center
Manager on results of outreach efforts and requests assistance when
needed. Promotes awareness of CCP’s Community Resource Center, both
brick and mortar and the virtual site to the community. Identifies
potential resources and events to promote on CCP’s Community
Resource Center website and in the physical center. Monitors
outreach outcomes and evaluates engagement data to support
continuous improvement initiatives. Performs other related duties

Qualifications: High School Diploma or GED with at least two years
of relevant experience, which may include, but not limited to:
customer service, non-profit organization, peer support, medical
office, behavioral health, or substance use program. Demonstrated
knowledge of and engagement with the local community preferred.
Bilingual Required (English/Spanish) Basic computer proficiency and
ability to document interactions in electronic systems. Skills and
Abilities: Excellent communication and interpersonal skills,
including active listening, the ability to speak using clear and
plain language and resolving conflict when necessary. Demonstrates
cultural awareness and provides respectful service to individuals
from diverse backgrounds. Must be able to work in a fast-paced
environment Skilled in maintaining good rapport and collaborative
working relationships with team members. Ability to travel within
the assigned community service area. Must be able to work safely in
community environments and follow established safety protocols.
